For a patient’s first consultation a long appointment is required to allow time for us to take your complete medical history and assess your individual needs.

Please arrive 10 minutes prior to your appointment to complete your new patient form, alternatively this can be completed in advance.

NEW PATIENT FORM

For a chronic disease management plan you will need to arrange an appointment with the Practice nurse to organise the paperwork and appropriate checks for your government-funded Podiatry, Dietetic, Physiotherapy and many other services. In the first instance, place speak to your GP to discuss your eligibility and health goals.

Childhood and other immunisations/injections, wound management, ECGs and other procedures need to be booked through the reception staff on 8349 9292. We require a GP assessment prior booking any procedures.

Sunday                         CLOSED – telephone appointments only

NB: The clinic is closed and not staffed on Saturdays & Sundays

Blair Athol Medical Clinic can offer telehealth appointments to exisiting patients of the clinic. There are some restrictions from medicare for medicare rebatable telehealth consulatations. In order to receive a rebate you must have attended the clinic face to face with the doctor in the last 12 months. If you have not attended the clinic in the last 12 months, then a full fee applies to the consultation with the doctor. Exceptions apply to children under the age of 12 months and those currently isolating due to COVID-19 illness. Please refer to the medicare MBS website for further information. Fees apply for telephone appointments outside of normal hours and weekends.

Blair Athol Medical Clinic is a Bulk Billing Clinic. Bulk billing is available Monday to Friday 8AM to 6PM. Weekends will incur a $20 gap fee, this includes telephone appointments, Procedures will also incur extra charges which must be paid on the day of your procedure.  Please ask our staff for more information. All accounts must be paid on the day of service, failure to do so may impact your access appointments.

Iron infusions – gap of $120
Mirena IUD insertion – gap of $100  (concession $80)
Implanon insertion – gap $50 (concession $40)
MS-2 Step – gap $50-100 each, 3 appointments

Please note that Iron Infusions require a doctor review prior to booking an iron infusion to ensure that it meets medical indications. We do not do new patient appointments for Iron infusions.

Our clinic has an Audiologist available to assess hearing loss and other hearing disorders. Speak to our reception staff for more information.
